Persistent Systems has announced its latest partnership with Microsoft to launch a Generative AI-powered Population Health Management (PHM) Solution. The solution will identify Social Determinants of Health (SDoH) from Electronic Health Records (EHR) data, enabling personalised care recommendations and cost-effective interventions that could lead to a 60% reduction in hospital readmissions within 30 days.

The PHM Solution released by Persistent Systems uses Microsoft Azure OpenAI Service and aims to ensure patients receive quality care at the right time and place while optimising both capacity and cost-effectiveness for healthcare providers and organisations. This innovative method utilises predictive risk modelling as a crucial tool to identify patients who are likely to require significant medical assistance.

The solution uniquely distinguishes itself by accurately extracting up to 80% of essential patient information from unstructured clinical notes contained within EHRs. Existing PHM solutions on the market have primarily depended on data from third parties or survey data. According to the announcement, Persistent's method aims to improve patient outcomes, offering more timely and intelligent care recommendations and boasting higher accuracy, improved scalability, and increased compliance.

Further, Persistent’s PHM solution has features that assist physicians and caregivers in recommending personalised interventions through community-based programs and care management teams. These features aim to reduce hospital stays, readmissions, and visits to the emergency department. The solution has the potential to decrease 30-day readmissions by up to 60%, and continuously monitor correlations and outcomes to ensure quality patient engagement.
